Linking Families & Individuals to Health Insurance, Benefits & Community Resources

At the Community Wellness Program, receive information and assistance in applying for many public benefits. Some public benefits are available regardless of immigrant status. Citizenship applications will not be affected if you receive health insurance or food benefits for which you are eligible.

In addition to those services listed below, we can provide information on many other public benefits and community resources.

At the Community Wellness Program we try our best to accommodate walk-ins, but it is best to schedule an appointment by calling (773) 523-5079 before coming to our facility.

Illinois All Kids - Healthcare For Kids

Health insurance for pregnant women and children under the age of 19.

The All Kids program offers Illinois uninsured children comprehensive healthcare that includes emergency room visits, doctors visits, hospital stays, prescription drugs, vision care, dental care and medical devices like eyeglasses and asthma inhalers. Parents pay monthly premiums for the coverage, but rates for middle-income families are significantly lower than they are on the private market.

You may qualify even if you already have insurance through your employer.

Benefits are available to children and pregnant women regardless of immigration status.

FamilyCare Illinois

FamilyCare offers healthcare coverage to parents living with their children 18 years old or younger. FamilyCare also covers relatives who are caring for children in place of their parents.

Like All Kids, FamilyCare covers doctor visits, dental care, specialty medical services, hospital care, emergency services, prescription drugs and more.

If you have health insurance and your family income qualifies for FamilyCare Share, Premium or Rebate, you have a choice. FamilyCare Rebate helps to pay for your insurance premiums. FamilyCare Share and FamilyCare Premium provide a medical card to help cover medical services that your plan does not cover.

Parents can get FamilyCare if they live in Illinois and meet the FamilyCare income limits. They must be U.S. citizens or meet immigration requirements.

SNAP (LINK Card / Food Stamps)

Formerly known as “Food Stamps” and also referred to as the LINK card, SNAP (Supplemental Nutrition Assistance Program) helps people with little or no income buy nutritious food.

To qualify, you must be a U.S. citizen or legal permanent resident of 5 years or more, and meet income requirements.

Children who are U.S. citizens or legal permanent residents can still receive assistance even if their parents don’t qualify. You do not need to have children to qualify for SNAP.

Free Pregnancy Tests

Saint Anthony Hospital Free Pregnancy Test

We offer free pregnancy testing at the Community Wellness Program for women who think they might be pregnant, or who need verification of pregnancy in order to apply for public health insurance.

No appointment is necessary for pregnancy testing.
